In preparation for your Tele-Boarding visit, please review your child's medical history and food intake form since it will be reviewed during this visit. Please also come prepared with any questions you may have about the program.
Tele-Boarding is a telehealth visit supported by Doc Response that will take place via video call, 4 weeks before your On-Boarding visit. You will receive a text message and/or email from Doc Response with the link to join 15 minutes ahead of your call. You will not need to download any apps for this call.
During this video call we collect your child’s medical records and discuss their medical history. This is also an opportunity to ask one of our physicians questions about the program.
Please make sure your child is present for the call, you have a stable internet connection, good lighting, and that the video and sound are working on your computer prior to the call. We will send you a reminder link 15 minutes before the video call.
